Membership begins after completion of a Comprehensive New Patient Visit to ensure a thorough assessment and a clearly defined care plan before ongoing care begins.

Membership Terms

​

Membership requires an initial three-month minimum commitment.

Membership renews monthly after the initial commitment period.

A Comprehensive Visit with Care Your Way Health within the past 12 months is required prior to enrolling in membership.

The New Patient Comprehensive Visit is not included as part of the membership.

Membership may be cancelled after the initial three-month commitment.

Membership fees are non-refundable once a billing cycle has begun.
